Cortisol vs. Testosterone: The Hormonal Tug-of-War Most Men Do Not Know About

If you are a man over 30 who wants to support your energy, recovery, and overall vitality, your first instinct is probably to jump straight to the testosterone conversation. That makes sense - testosterone gets all the attention. But there is a quieter, upstream problem that rarely makes the headlines: chronic stress is actively working against your testosterone, and it has been doing so for years.
This is the cortisol-testosterone relationship, and once you understand it, you will think about hormonal health differently.
The Hormonal Tug-of-War
Cortisol and testosterone share a common precursor: cholesterol. Your body converts cholesterol into pregnenolone, which then branches into two pathways. One leads to cortisol (your primary stress hormone). The other leads to testosterone and other sex hormones.
The catch: when you are under chronic stress - work pressure, poor sleep, overtraining, financial strain, or just the accumulated weight of modern life - your body prioritizes cortisol production. It has to. Cortisol is part of the survival response. Your body will always choose stress management over reproduction when resources are limited.
This creates what researchers sometimes call "pregnenolone steal." Your body diverts more pregnenolone toward cortisol production, leaving less raw material available for testosterone synthesis. Cortisol wins by default, because your body treats it as more urgent.
The relationship is well documented. High cortisol correlates with lower testosterone. When cortisol stays elevated for extended periods, testosterone production takes a hit - not because anything is wrong with the testosterone pathway itself, but because the upstream resources are being redirected.
This is why stress management is not just a wellness nicety. It is a hormonal strategy.
What Chronic Stress Actually Does
Acute stress is not the issue. A hard workout, a tight deadline, a cold plunge - these spike cortisol briefly, and your body recovers. That is healthy. That is the system working as designed.
The problem is chronic elevation. When cortisol stays high for weeks and months, the downstream effects compound:

Impaired recovery. Cortisol is catabolic - it breaks tissue down. When it stays elevated, your body struggles to repair and build. This is why overtraining (a form of chronic physical stress) often backfires.
Disrupted sleep. Cortisol should follow a natural rhythm: high in the morning, low at night. Chronic stress flattens that curve, keeping you wired when you should be winding down. Poor sleep then further suppresses testosterone, creating a vicious cycle.
More belly fat, less testosterone. Elevated cortisol promotes visceral fat storage, particularly around the midsection. Visceral fat contains aromatase, an enzyme that converts testosterone to estrogen. So more belly fat means more testosterone gets converted away. This is a well-documented biological relationship, though individual hormonal profiles vary and should be discussed with a healthcare provider.
You can see how this spirals. Stress raises cortisol. Cortisol suppresses testosterone. Lower testosterone impairs recovery and sleep. Poor recovery and sleep increase stress. The cycle feeds itself.
Breaking it requires addressing cortisol directly, not just chasing testosterone numbers.
KSM-66 Ashwagandha: Addressing the Source

Ashwagandha is classified as an adaptogen, a compound that helps your body resist and recover from various stressors. It does not work by sedating you or masking stress. They help your body calibrate its cortisol output more appropriately rather than staying stuck in overdrive.
The most cited study on this is Chandrasekhar et al. (2012), a randomized, double-blind, placebo-controlled trial in 64 chronically stressed adults. Participants taking KSM-66 ashwagandha showed a 28 percent reduction in serum cortisol from baseline (Chandrasekhar et al., 2012). That is a substantial, measured change in the hormone that directly competes with testosterone for resources.
The same research group found significant improvements in scores on standardized stress questionnaires, suggesting the cortisol reduction translated to how people actually felt - not just lab numbers.
On the testosterone side, a pilot study in subfertile men found that KSM-66 supplementation over 90 days produced a 17 percent increase in serum testosterone (Ambiye et al., 2013). This was in men with fertility issues, so the effect in healthy men would likely be smaller. A separate RCT in overweight men aged 40 to 70 found approximately 15 percent higher testosterone versus placebo, though that study used a different ashwagandha extract (Lopresti et al., 2019).
There is also the exercise performance data. An 8-week randomized, double-blind trial found that the ashwagandha group achieved significantly greater strength gains versus placebo, with the researchers noting increased testosterone in the supplementation group (Wankhede et al., 2015).
The strongest evidence arrived in 2026: a meta-analysis pooling 23 randomized controlled trials (1,706 participants) confirmed both sides of this picture at once - ashwagandha significantly lowered cortisol (pooled SMD -1.18) and raised testosterone in men by an average of 57 ng/dL, with no effect in women (Hormonal Modulation meta-analysis, 2026). Pooling dozens of trials this way is far more reliable than any single study - it puts the individual findings above on firm footing.
What makes ashwagandha compelling is this dual action. It may help lower the hormone that suppresses testosterone (cortisol) while supporting testosterone parameters directly. It is not a testosterone drug. It is an adaptogen that helps create the hormonal conditions where testosterone can function normally.
Tongkat Ali: A Complementary Pathway
Ashwagandha works primarily through the stress-cortisol axis. Tongkat ali (Eurycoma longifolia) comes at testosterone support from a different angle.
Much of your testosterone is bound to sex hormone-binding globulin (SHBG), which renders it inactive. Only "free" testosterone - the unbound portion - can attach to androgen receptors and exert effects. What matters for how you feel is often free testosterone, not total testosterone.
One proposed mechanism for tongkat ali is reducing SHBG binding, increasing the proportion of testosterone that is free and available. This is a different lever than what ashwagandha pulls. Ashwagandha may help with production conditions. Tongkat ali may help with utilization.
A study in moderately stressed adults found that tongkat ali supplementation significantly improved the testosterone-to-cortisol ratio, with salivary testosterone increasing by 37 percent and cortisol decreasing by 16 percent versus placebo over four weeks (Talbott et al., 2013). Worth noting: testosterone was measured via salivary assay (not the more precise serum test) in a mixed-sex sample, so the numbers should be interpreted with that context.
In an open-label study of men with low-normal testosterone, tongkat ali supplementation was associated with improved testosterone parameters in most participants (Tambi et al., 2012). Open-label means participants knew what they were taking, which introduces bias. But the direction and consistency of response in this population is notable.
The evidence for tongkat ali is encouraging but not as strong as ashwagandha's. Study quality varies, and effects may be more pronounced in men who are already stressed or have low-normal testosterone. We include it because it works through a mechanism that complements the other testosterone-supporting ingredients in the formula rather than duplicating them.
Breaking the Cycle
The real insight here is that hormonal health is not about any single number. It is about the relationship between systems.
If cortisol is chronically elevated, simply "boosting testosterone" through one pathway may not be enough. The upstream drain is still there. You are filling a bucket with a hole in it.
